Does anyone have examples of good policies for self insured companies particularly in banking?

Wells Fargo has a policy. I helped a family with Wells get the benefit. It is run through Optum Behavioral Health, which is tied into UBH.
I tried to start a JetBlue Airways campaign by filling out all of the necessary blocks but seem to have trouble getting it to show up or generating a website like the other companies have.
